REQUIRED FORMS FOR  SCHOOL YEAR 2010-2011

Required forms to be returned to the Seton main office as soon as possible and no later than the first day of schoool - August 25, 2010.  Please print out, complete, sign, and return the following SEVEN FORMS:

  1. Educational and Conduct Contract 2010-2011 (acknowledging that you have read, understand, and agree to abide by the contents of the 2010-2011 Seton Catholic School Family Handbook)
  2. Legal Custody Form (pdf)
  3. Photo Release Form (pdf) 
  4. Family Directory Permission Form (pdf)
  5. Extended Day Program Registration Form (pdf) (please complete this form if you intend to use this service at any time during the year - on a regular, occasional, or emergency basis)
  6. Request for Administration of Medication by School Personnel Form (pdf)  (optional - form to be completed if any medications are to be administered.)
  7. Volunteer Bus Duty Form for 1st Week of School (pdf)  (optional - form to be completed by those willing to volunteer for this first week of school duty)

In addition to the above forms each family will receive an envelope (to be picked up at the Parent Orientation Meeting on August 23, 2010) with three completed forms that will require verification and updating as needed.  All website and 'picked up' forms can be returned in the family envelope to the main office on or before August 25, 2010.  Thank you!
